Lifetime Active Membership Application

Please use this form to become a Lifetime Active Member of Lambda Theta Alpha Latin Sorority, Incorporated. <ul> <li>Applications will be accepted at any time throughout the year.</li> <li>The fee for Lifetime Membership is $710.</li> <li>You will be asked for:</li> <li>Proof that you have been a member for a minimum of 10 years. Sample documentation you can provide includes a Sister Certificate, Email from Initiated Chapter President or Local Leadership, Chapter/Area Sister Directory.</li> <li>A high quality, passport size head shot to be used on your lifetime membership card *. Photo should only be of your head including your shoulders, looking forward into the camera.</li> <li>Please allow approximately 30 days for processing of your application.
